Abstract
Cerebralna paraliza jedan je od najčešćih oštećenja kod djece. Glavni uzroci i čimbenici rizika kod cerebralne paralize jesu: višestruki porod, ekstremna nedonoščad, problemi s hranjenjem, produljena hospitalizacija ili postnatalna infekcija. Najučestaliji tip cerebralne paralize je spastični tip. Pogađa oko 75% djece. Gruba motorička funkcija djece i mladih s cerebralnom paralizom može se kategorizirati u 5 različitih razina pomoću alata nazvanog gross motor function classification system. Neurorehabilitacija je ključna stavka kako bi rehabilitacija djece bila što uspješnija. Neurorehabilitacija se po mnogo stvari razlikuje od ostalih grana neurologije. Rehabilitacija je proces edukacije osobe s invaliditetom kojem je krajnji cilj pomoći pacijentu da se samostalno nosi sa svakodnevnicom kao što su posao, obitelj, prijatelji i slobodno vrijeme. Najveći značaj na način liječenja i rehabilitacije cerebralne paralize prepisuje se B. i K. Bobath i V. Vojta. Oni su svojim metodama uvelike pridonijeli kvaliteti liječenja pacijenata. Kao nova metoda rehabilitacije djece s neurološkim oštećenjima pojavljuje se terapija temeljena na robotima. Ova rehabilitacijska terapija potpomognuta robotom sastoji se od mehatroničkog uređaja koji autonomno potpomaže repetativnom izvođenju specifičnih pokreta. Korištenje robota u rehabilitacijskim terapijama donosi prednosti u odnosu na tradicionalne terapije. Istraživanaj pokazuju da roboti u fizioterapiji ne koriste samo pacijenata već i samanjuju opterećenje na mišićno-koštani sustav terapeuta. Roboti za rehabilitaciju često se klasificiraju prema njihovoj mehaničkoj strukturi i općenito se dijele na krajnje efektore i egzoskelete. Lokomat je robotski uređaj koji je dizajnirala Hocoma AG za liječenje odraslih i djece. Lokomat se sastoji od egzoskeleta s 2 ortoze s motornim pogonima, potporom za tjelesnu težinu i sinkroniziranom trakom za trčanje. Studija razvijena sa 16 korisnika koji su prethodno imali moždani udar pokazala je da su nakon liječenja svi pacijenti poboljšali performanse hoda i motoričke funkcije. Uređaj za neuromotoričku rehabilitaciju gornjih ekstremiteta. Gloreha Sinfonia je najnaprednija verzija Gloreha uređaja za rehabilitaciju gornjih udova. Provedena istraživanja dokazuju da gloreha robot ima pozitivne učinke na funkciju distalnog djele gorenjeg uda kod oboljelih od cerebralne paralize.
Abstract (english)
Cerebral palsy is one of the most common lesions in children. The main causes and risk factors for cerebral palsy are: multiple births, extreme prematurity, feeding problems, prolonged hospitalization or postnatal infection. The most common type of cerebral palsy is the spastic type. It affects about 75% of children. Rough motor function of children and adolescents with cerebral palsy can be categorized into 5 different levels using a tool called gross motor function classification system. Neurorehabilitation is a key item to make the rehabilitation of children as successful as possible. Neurorehabilitation differs in many ways from other branches of neurology. Rehabilitation is a process of educating a person with a disability whose ultimate goal is to help the patient cope with everyday life such as work, family, friends and leisure. The greatest importance in the way of treatment and rehabilitation of cerebral palsy is attributed to B. and K. Bobath and V. Vojta. They have greatly contributed to the quality of patient care with their methods. Robot-based therapy is emerging as a new method of rehabilitating children with neurological impairments. This robot-assisted rehabilitation therapy consists of a mechatronic device that autonomously assists in the repetitive performance of specific movements. The use of robots in rehabilitation therapies brings advantages over traditional therapies. Research shows that robots in physiotherapy not only benefit patients but also reduce the burden on the therapist's musculoskeletal system. Rehabilitation robots are often classified according to their mechanical structure and are generally divided into end effectors and exoskeletons. Lokomat is a robotic device designed by Hocoma AG for the treatment of adults and children. The locomotive consists of an exoskeleton with 2 orthoses with motor drives, weight support and a synchronized treadmill. A study developed with 16 users who had previously had a stroke showed that after treatment, all patients improved gait performance and motor function. Device for neuromotor rehabilitation of the upper extremities. Gloreha Sinfonia is the most advanced version of Gloreha upper limb rehabilitation device. Studies have shown that the gloreha robot has positive effects on the function of the distal part of the upper limb in patients with cerebral palsy.
